An Observation is an event that has the potential to cause injury or damage. Submitting an Observation Report means we can remove immediate risk, and understand strategically where we need to make improvements by identifying areas of high risk and taking steps to prevent a more serious incident from occurring.Submitting your name and contact details are optional, however your identity will remain confidential.
